RELEASE, WAIVER AND INDEMNITY
By choosing to participate in a Creative Dreamwork workshop, retreat, class, or private session, I accept full responsibility for any risks to myself—physically, psychologically, or materially—including but not limited to accident, injury, illness, loss, or damage. I acknowledge that I am participating voluntarily and at my own discretion.
I understand that this is an educational program. The intention of the program is to support creativity through the practices of Creative Dreamwork.
1. Release of Liability
I, on behalf of myself, my heirs, executors, successors, and assigns, hereby release, waive, and forever discharge Creative Dreamwork, Kim Gillingham, and any associated individuals, venues, or organizations from any and all claims, demands, damages, expenses, actions, and causes of action—whether in law or equity—arising from any injury, illness, loss, or damage to person or property, however caused. This release applies to all incidents occurring before, during, or after my participation in the program, even if caused in part by negligence.
2. Medical Consent
In the event of accident, injury, or illness, I consent to receive necessary medical treatment during my participation in the program.
3. Confidentiality Agreement
I agree to maintain the confidentiality of all personal content, scripted material, third-party material, and images shared by others during the course of the program.
